Requirement:
- The application form duly completed along with the Spectrum Licence fee;
- SMA Applicant Vetting/Address verification form (duly completed)
- Two recent photographs (2½” x 2” or less than 2” x 1½”) to be taken full face without hat and must not be mounted. The reverse of each photograph must be certified by any Resident Magistrate, Justice of the Peace, Minister of Religion, Collector of Taxes, Clerk of the Courts, Police Officer (Gazetted Rank), Bank Manager, Medical Practitioner, Solicitor of the Supreme Court of Barrister-at-Law who is a Commonwealth Citizen, ordinarily resident in Jamaica;
- The required qualification, that is, Operating Certificate in Radiotelephony or Radio-telegraphy held by the applicant.
